Position Summary
To manage health systems and records in the PACT supported sites making sure all reports are accurate and timely.

Duties & Responsibilities
- Updating of data management tools including registers based at the CCC and MCH in the health facilities.
- Updating files and opening of new files for new clients
- Management of the filing system at the CCC i.e. filing and retrieval.
- Data reconstruction and computerization from source documents as required
- Data entry of routine data into the IQ Care database
- Data quality checks against client data from medical records
- Flagging of files for CD4, counseling, eligibility and starting of ARVs
- Compilation of the monthly facility reports (e.g. MOH 711) and other required reports
- To ensure that all patient encounter forms in the files assigned to him/her are accurately and completely entered into respective forms in IQ Care.
- To conduct data quality checks and audits on data entered into IQ Care and share with the respective supervisor.
- To work across different sites and ensure no backlogs of data.
- Notify the relevant clinical team in case of omissions, errors or ambiguous entries in the forms for updating and corrections to enhance accurate documentation.
- Reconstruct information from blue card into IQ Care where patient encounter forms are missing but blue card is available and complete.
- Ensure the file has been a valid number and is filed back into its filling place. Ensure the files have respective tracer cards where applicable.
Qualifications
- College Diploma with a specialization in Health Records and Information, Information Technology, Computer Science, Information Systems, Public Health Informatics, Statistics or Mathematics.
- A minimum of two (2) years’ experience handling health records, statistical data or population/ demographic data or information.
- Good computer skills on Ms Office applications especially Ms Word, Ms Excel and Ms Access. Ms PowerPoint.
- Knowledge of data management principles.
